On Thursday 14th February 2019, our CardioWellness4Slough team are attending the sixth annual Cardiovascular Disease Prevention Conference in Manchester, hosted by Public Health England (PHE). The focus of this year’s conference is on scaling up CVD prevention to improve service impact and health care outcomes.
Solutions 4 Health will be presenting a poster on our Integrated Cardiac Disease Prevention Programme. This focused on our CardioWellness4Slough integrated service which was designed to provide and coordinate early help for residents. This is achieved through better and faster access to information, advice and guidance, using health risks assessments and onward referral to appropriate services. Through this service, results have shown that integrating lifestyle services is fundamental in improving health and wellbeing.
